Please read the following before beginning the registration process for your middle school student.

Below is a list of all the required paper work for middle school athletes. If it says that the a particular piece of paperwork is required each season you must turn in a copy of this paperwork for the spring season even if you have completed it for the winter and/or fall season.

If a particular piece of paperwork says that it is required yearly then it must be completed eachschool year, so individuals who completed it for either last winter's or last spring's sports seasons must complete it once more and pass it in again. For individuals who completed it for this fall and/or winter 2012 season, please email iacsathletics@innovationcharter.org with your student-athletes name as well as your own name and inquire as to the whether or not the paperwork in question is on file. Unfortunately, some of the paperwork may not have been returned by the league. Please allow 48 hours for a response.

For anything that is required to be on file with the Athletic department (birth certificate), please email iacsathletics@innovationcharter.org if you want to double check that it is on file with the Athletic Department. Some students who are new to this school do not have a birth certificate on file. Please allow 48 hours for a response.

There is a $125 athletic fee. IACS understands that this may be too much of a burden for some families to pay. Please do not let this fee prevent your child from participating in a sport for their school. If you need any financial assistance please just contact Nolan McKinnon at nmckinnon@innovationcharter.org and arrangements will be made.

Please read the requirements of the student-athlete's physical as it is a new policy that started last season. Any questions please email iacsathletics@innovationcharter.org

Below are all of the required forms for middle school registration. Please make sure that all 10 requirements have been met and all paperwork is turned into the athletic department
before Thursday February 28, 2013.

**** It is now required by the league that both a parent and student-athlete have separately                 completed concussion certifications. This means that both parent and student must have separate       log-ins at NFHS and the two certifications must have different names typed on them. The league no     longer allows us to have the student-athlete sign that they were present. We apologize for any             inconvenience, but once again this is required by the league. ****
