Reflection Form for Recommendation (Athletic Administration)
If any student wishes for a college, work, or personal recommendation from an athletic administrator they should first fill out this form. They should then give this completed form to the athletic administrator when they ask for the recommendation.

If any student wishes for a college, work, or personal recommendation from an athletic coach they should first fill out this form. They should then give this completed form to the coach when they ask for the recommendation.
Athletic Department Recommendation Agreement
In order to help each junior get organized in the college application process, we are asking that you identify a coach or athletic office staff member who could write a positive recommendation regarding your high school athletic career. You may ask someone, who has coached or worked with you at some point during your high school athletic career and could speak to your character as an athlete. This does NOT take the place of a teacher recommendation, however.
Athletic Fee Reimbursement Form
If for any reason you would like a refund please fill out the below form and return it to the Athletic Department. The form will be reviewed and a decision about the reimbursement will be made within two business days of the receipt of the form. You may be granted a full, partial, or no reimbursement depending on the circumstances surrounding the request.
Athletic Financial Assistance Form
If financial assistance is required, please click below for a financial assistance form. If needed, complete the form and submit it with your paper work to the Athletic Department. Innovation Academy Athletics believes that no student should be excluded from the opportunity to participate in athletics for financial reasons.
IACS Athletic handbook
Each summer using input from staff, parents, and students, the Innovation Academy Athletic Handbook is updated. We ask that all parents and student go through the handbook. High School families will agree to the terms of the handbook online through FamilyID, but should completely read through the handbook as well. It is important to be familiar with the handbook as all student-athletes will be held accountable to its standards once signed online. If you have any clarifying questions or concerns about the handbook, please set up a meeting with the Athletic Director, Nolan McKinnon, so that your concerns may be addressed properly. A student will not be allowed participate until a signed handbook is on file.
